Outcomes-based chronic care

At AmeriHealth Caritas, we work to address care gaps and foster health equity with a proven integrated care management (ICM) model. This model incorporates a member-based decision support system that drives both communication and care-plan development through a multidisciplinary management approach.

Our comprehensive chronic care programs effectively address the complex chronic conditions of Medicaid recipients. These include people with asthma, diabetes and heart failure, plus chronic obstructive pulmonary disease (COPD), sickle cell anemia and other disorders identified by the Department of Health and Hospitals (DHH). AmeriHealth Caritas' programs have a successful track record of improving health outcomes and reducing preventable hospital admissions.

Breaking down barriers

An AmeriHealth Caritas Louisiana case manager received a referral from Rapid Response Team (RRT) for case management outreach in Baton Rouge, LA.

The member initially called RRT because of a barrier to transportation. An in-depth assessment uncovered the member's diabetes compliance issues. Her glucometer didn't work, plus she lacked adequate eye care and couldn't afford new $140 eyeglasses.
