Because we have worked with Medicaid members for 30 years, we know it is important to identify, understand, and address interrelated chronic conditions. Our integrated holistic approach to care does just that—with excellent results nationwide.

Medicaid members with chronic illnesses often have multiple co-morbid conditions. Frequently, they have behavioral health issues and lack an adequate support system. AmeriHealth Caritas' Integrated Care Management model connects case and disease management in a holistic approach that addresses co-morbid conditions, behavioral health needs and social/environmental support needs. We then combine all condition-specific assessments for multiple co-morbidities into one care plan that supports body and mind.

Condition management blueprints guide our care managers for each co-morbid condition. These evidence-based blueprints are available for asthma, diabetes, chronic obstructive pulmonary disease (COPD), heart disease, sickle cell anemia and depression.

Medication management for a normal life.
Anthony is an 11-year-old boy with asthma. His asthma was so severe that he had multiple attacks when his asthma attacks would not subside, resulting in frequent emergency room visits and hospitalizations. The boy's doctors wanted him to try a specialized injectable medication, Xolair. Although the medication was not on the health plan's formulary, Anthony's doctors worked with the plan's medical directors to have the medication approved for him. After five months on Xolair, he had no further emergency room visits or hospitalizations, and his fragile health improved to the point where this home-schooled child was well enough to attend school again.
